I'm sure that no one is perfect, not even huge companies, but I wonder how Toyota got to the point of not monitoring disk space.

They were doing database maintenance - it's not uncommon to need 2 to 3 times free disk space as the size of the database with some systems. Not that I have any personal disastrous experience with such things :p

Taking the JIT philosophy a little too far I see.

You mean they couldn't get a just in time delivery of a few more gigabytes?!?
