It was not a glitch: it was faulty software. The same company (Fujitsu) were supposed to supply an integrated patient data system in the UK, but spectacularly effed up, and the system was cancelled. Fujitsu then successfully got compensation when it was cancelled.

Not too mention it was originally a billion pound project. People need to go to jail!
